This text matches up with the SMC SMCUSBH7, which is pretty much identical in form factor, identical in back port labeling, and identical in LED configuration. The manufacturer of the device (which seems to be Good Way [pdf]) is certainly the same.

This device is presumed to also use ISP1521BE, which is used by the SMCUSBH7.

The power adapter included with the device may be a DVE DSA-0131F-05 US 13 (100-240VAC ~ 50/60Hz 0.3A -> 5V 2A).
