This device looks quite similar to the Belkin F5U014 or Belkin F5U014-OE. They may share the same manufacturer (perhaps.. but the F5U014 seems to be manufactured in Taiwan) or the manufacturer is simply copying the same design.
The text on the box front reads "Blue Triangle" (under a picture of a butterfly), "USB Hub", "1 Upstream port", "4 Downstream ports", a USB logo, and "Genie" (USB Genie?).
"AT114D-0202" is silkscreened on the board.
- The Atmel Hub IC is marked with "ATMEL 0120", "AT43301/55634D", "R0C1403".
The manual indicates the model is USB-114, however, the sticker on the bottom of the hub reads AS-2001. There appears to also be a (6-port) AS-2000 hub from this manufacturer / vendor.
The power adapter included with the device appears to be a JODEN JOD(M)-SW-015 (100~127VAC @ 60Hz -> +5VDC 2.5A).
