This device is clearly a Cameo WLG-1202.
"NETGEAR" and "32-bit Wireless PCI Adapter Rev.A3" (as well as a large bit of FCC compliance text, some other stuff, the FCC ID, and the IC ID) is silkscreened on the board. A label on the back of the board identifies the device as a WG311T V1H4 (though it seems this device can also have a label identifying it as an WG311T V1H3 [1]).
